Thank you for taking the time to submit a story.
This is our guide for providing us with all the information we’ll need to put together your story for the CWC website, which may be published so everyone can witness your incredible journey as we walk with you.

What you’ll need:
Your grant application answers will be referenced in some of the questions below. Please have it readily available to effectively share your story. Specific questions from the grant will be in parentheses for your convenience (i.e. Question 2). Please also ensure you write your answers in a backup document and then place them in the form to avoid losing your work.

What is the main objective of your program?

The name of your community partner and executive director (question 4)

A short description of your community partner (question 2)

Brief history of your community partner's work in this area (question 4)

What have you learned so far? What has surprised you or stood out in your experience?

What difference is this grant making in your community?

Please share 3-5 measurable outcomes (e.g. attendance, meals served, volunteer hours)

If your program did not exist, what would your community look like?

What does receiving a CWC grant mean to you?

How has your interaction with CWC supported your work?

We LOVE to see photos and videos of the program in action. When taking photos, make sure your flash is on (even in a bright environment). If you’re using a phone or tablet, turn the device to landscape when taking photos or videos. Put all photos and videos together in an online gallery using a cloud service (e.g. Google Drive, Microsoft OneDrive, Drop Box, iCloud, Amazon Photos, Flickr, etc.). Grant us access, and place link in the box below.
